I wrote this eBook because immediate loading changed the way I practice implant dentistry.
When I started placing implants, the conventional protocol was clear: place the implant, wait three to six months, and then restore. It was safe, predictable, and well-documented. But it was also slow, and patients often spent months with removable prostheses or gaps in their smile. That experience shaped my interest in finding faster, evidence-based alternatives.
Immediate loading, when performed under the right conditions, offers patients a fixed prosthesis within 48 to 72 hours of implant surgery. The clinical benefits are obvious: fewer surgical interventions, faster return to function, improved patient satisfaction, and in many cases, better soft tissue outcomes.
But immediate loading is not without risk. It demands careful patient selection, adequate primary stability, appropriate implant design, and meticulous prosthetic management. This book aims to provide the clinical knowledge and practical guidance needed to apply these protocols safely and predictably.
Dedication
This book is dedicated to my wife Maria and our three sons, who have always supported my clinical and academic work. To my parents, Manuel and Maria, who taught me the value of discipline and perseverance.
I am deeply grateful to Prof. Dr. Jose Maria Suarez Quintanilla, who guided my academic journey, and to Prof. Dr. Ramon Lorenzo Vignau, who taught me most of the implant techniques described in this book.
Special thanks to Mr. Daniel Carmona, who introduced me to the digital world and whose CAD/CAM expertise made many of these cases possible. And to Diogo Luis Alves, who contributed extensively to the literature review and clinical cases.
Overview of Immediate Loading
Immediate Loading Overview: Advanced Implantology Workgroup
Key Definitions
Before we begin, it is essential to understand the terminology used throughout this book:
- Immediate Restoration: A prosthesis is placed within 48 hours of implant surgery, but with no occlusal contacts
- Immediate Loading: A prosthesis is placed within 48 hours of implant surgery, with occlusal contacts
- Early Loading: A prosthesis is placed between 48 hours and 3 months after surgery
- Conventional Loading: A prosthesis is placed after 3 to 6 months of healing
- Progressive Loading: A gradual increase of occlusal load over time
The distinction between immediate restoration and immediate loading is crucial. While both involve placing a prosthesis within 48 hours, the presence or absence of occlusal contacts fundamentally changes the biomechanical demands on the implant-bone interface during the healing period.
Throughout this book, we will explore the biological foundations that make immediate loading possible, the clinical parameters that determine success, and the step-by-step protocols for both single-tooth and full-arch cases.
